Notice of Infringement – Claim

- A statement that the information in the notification is accurate and, under penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.
Please note that, under Title 17 U.S.C. §512(f), anyone who knowingly submits false claims of infringement may be held liable for damages, including costs and attorney’s fees.

Counter Notification – Restoration of Material
If you believe that content was removed or disabled by mistake or misidentification, you may send a counter-notification in accordance with 17 U.S.C. §512(g)(3). The counter-notice must include:
- Your physical or electronic signature;
- Identification of the material that was removed and its original location before removal;
- A statement under penalty of perjury that you have a good faith belief the material was removed or disabled due to error or misidentification;
- Your name, address, and telephone number;
- A statement that you consent to the jurisdiction of the federal district court for your judicial district (or if outside the United States, to any judicial district in which the Site may be found).
